How to Request South Jordan Death Certificates

South Jordan residents have three options for requesting a certified death certificate. The fastest is an in-person visit to the South Redwood Public Health Center or another county health office. Mail requests and online orders are available for those who cannot visit in person.

For in-person visits, bring a valid, unexpired government-issued photo ID. A driver's license, state-issued ID card, U.S. passport, military identification card, permanent resident card, or other county-approved form is acceptable. Also bring a completed application and proof of your relationship to the deceased. Payment at the counter is accepted by cash, check, money order, Visa, or Mastercard.

Mail requests require a fully completed application, a clear photocopy of your photo ID, and clear photocopies of your proof-of-relationship documents. Write a check or money order payable to the Salt Lake County Health Department. Credit cards cannot be processed for mail orders. Address the envelope to the health office and write "ATTN: VITAL RECORDS" clearly on the outside. The county returns your certified South Jordan death certificate by mail after processing.

Online orders are processed through VitalChek, the authorized online service for Utah vital records. VitalChek accepts credit cards. The fee is $30 for the first certified copy and $10 for each additional copy in the same order. You can also access the ordering page directly through saltlakecounty.gov/health/vital-records/order.

Since August 21, 2024, proof of relationship is mandatory for all Salt Lake County vital records requests. If you are the surviving spouse listed on the South Jordan death certificate, no additional proof is required beyond your own photo ID. Children of the deceased must show their own birth certificate naming the deceased as a parent. Siblings must provide a birth certificate showing at least one shared parent with the deceased. Grandchildren and grandparents must provide a chain of birth certificates showing the family connection.

Note: If no qualifying relative can provide appropriate ID and relationship proof, the county may accept documentation of a direct and legitimate interest in the record, such as legal guardianship papers or a court order.

South Jordan Death Records History

South Jordan was settled in the mid-1800s along the Jordan River. The area developed slowly as an agricultural community before urban growth accelerated in the 1990s and 2000s. Before South Jordan was incorporated in 1935, the area was part of unincorporated Salt Lake County. Early death records for the South Jordan area before incorporation appear under Salt Lake County designations in the historical archive.

Utah launched its statewide death registration system in 1905. For deaths from 1966 to the present, contact the Salt Lake County Health Department. Records more than 50 years old are available to any member of the public under Utah Code 26-2-22. Records from the last 50 years require a qualifying relationship or documented legitimate interest. South Jordan Death Certificate Fields

A certified South Jordan death certificate is an official state document produced under the authority of the Salt Lake County Health Department. It carries a raised seal or security printing and is accepted by government agencies, courts, financial institutions, and insurance companies as legal proof of death.

Standard fields include the full legal name of the deceased, date and place of death, time of death, date of birth, age, sex, race or ethnicity, marital status, Social Security number (post-1937 records), last residential address, father's name, mother's maiden name, informant's name and their relationship to the deceased, usual occupation, cause of death with immediate and underlying causes listed, contributing conditions, manner of death, name of attending or certifying physician, and place and type of final disposition.

South Jordan death certificates are used for life insurance and pension claims, estate administration, survivor Social Security applications, property title transfers, bank account closures, vehicle title changes, and genealogical documentation.
