Obtaining Herriman Death Certificates

Herriman death certificates are available through three channels. In-person requests are processed the same day. Mail requests are returned by mail after processing. Online orders through VitalChek are available for those who prefer electronic transactions.

To request in person, visit the South Redwood Public Health Center or another county health office during business hours. Bring a completed application form, a valid government-issued photo ID (driver's license, passport, military ID, or other acceptable form), and documents proving your relationship to the person named in the record. Payment by cash, check, money order, Visa, or Mastercard is accepted at the counter. Your certified Herriman death certificate will be ready the same day.

Mail requests to the Salt Lake County Health Department, ATTN: VITAL RECORDS. Include the completed application, a clear photocopy of your photo ID, clear photocopies of your proof-of-relationship documents, and a check or money order made out to the Salt Lake County Health Department. Credit cards cannot be accepted by mail. The county processes your request and returns the certified copy by mail.

Online ordering is handled by VitalChek, the state-authorized online service for Utah vital records. VitalChek accepts credit cards. The standard fee is $30 for the first certified copy of a Herriman death certificate, with $10 for each additional copy in the same transaction. Ordering through the county's ordering portal is also available.

Proof of relationship is required as of August 21, 2024. Spouses named on the certificate need only their own ID. Adult children must provide their birth certificate listing the deceased as a parent. Siblings must show a birth certificate with a shared parent. All other qualified parties must demonstrate direct and legitimate interest in the record.

Note: Herriman death records for the past 50 years require a qualifying relationship or documented interest. Records older than 50 years are available to any requestor without relationship documentation.

Herriman Historical Death Records

Herriman was settled in the late 1800s but remained a small rural community for most of its history. Before the city's rapid growth in the early 2000s, Herriman was part of the unincorporated Salt Lake County area. Death records for the Herriman area dating back to the earliest years of Utah state registration are part of the Salt Lake County archive.

The FamilySearch Utah Death Records collection includes records from Salt Lake County from 1904 through 1966. These are freely accessible and searchable by name. Standard fields on a Utah death certificate include: full legal name of the deceased, date of birth, date and place of death, age, sex, marital status, Social Security number (post-1937), last residence, father's and mother's names, informant name and relationship, cause of death, contributing conditions, manner of death, name of certifying physician, and place and method of final disposition. Modern certificates may also include education level and industry or occupation codes.
