Weber-Morgan Health Department Vital Records

The Weber-Morgan Health Department provides official, raised-seal birth, death, marriage, and divorce certificates for Weber County and Morgan County. The office is located on the first floor of the main Weber-Morgan Health Department building at 477 23rd Street, Ogden, UT 84401. Call (801) 399-7130 with questions about vital records. Weber-Morgan Health Department vital records office for Weber County death certificates

Starting August 21, 2024, proof of relationship is required to obtain any vital record from the Weber-Morgan Health Department. This applies to all request methods. Eligible requesters typically include immediate family members, legal representatives, and others who can document a legitimate need. Weber County Death Certificate Ordering Methods

The Weber-Morgan Health Department offers three ways to request a death certificate. Each method suits different needs and situations.

Same-day service is available for in-person visits to the Ogden office. Bring a completed application, a valid photo ID, proof of relationship to the deceased, and payment. Staff process most requests while you wait. This is the fastest option and avoids mailing delays. The office is on the first floor of the main WMHD building at 477 23rd Street.

Mail requests are also accepted. Send a completed application with a clearly legible photocopy of your valid ID, documentation of your relationship to the deceased, and a check or money order made payable to the health department. Do not send cash by mail. Allow three weeks before inquiring about the status of a mailed request. Mail to the Ogden address listed above.

Online ordering through SILVER, a secure online service from Utah.gov, is a third option. You can also order Weber County death certificates through VitalChek, which is an authorized ordering partner. Additional service charges apply for online orders. Certified copies are mailed to your address. This is the best choice for those who live outside Ogden or cannot visit during business hours.
