Requesting Taylorsville Death Records

In-person requests at any county health office give you a certified Taylorsville death certificate the same day. Walk in during business hours and go to the vital records counter. Present a valid, unexpired government-issued photo ID. A driver's license, state ID, U.S. passport, military ID, or other qualifying form is accepted. Online orders are handled by VitalChek, Utah's authorized online vital records service. VitalChek accepts credit cards and arranges delivery. The fee is $30 for the first certified Taylorsville death certificate and $10 for each additional copy in the same order.

Proof of relationship is mandatory as of August 21, 2024, for all Salt Lake County vital records requests. Spouses named on the certificate need only their own valid ID.
